I've even tried switching the console region to United States and it doesn't show up there either. What happens is that although the system will show Knights of the Nine and Shivering Isles installed, the game does not see you as owning the content and therefore no longer activates it (verified by checking the in-game DLC list) I have the other pieces of DLC showing up in-game that I bought way back in the Xbox 360 days but not Shivering Isles and KotN. Since I own the GOTY physical release of the game, I was able to pop in the second disc and have it download the Shivering Isles and Knights of the Nine DLCs.
